1992 was the last time American Standard & Kohler gave out their rough-in books. Both will send you an architectural binder that includes rough-in dimensions, if you ask for them.

You will not find one. The plumbing layout will depend on the location of the fixtures, and the point of exit from the building. If you were to give the blueprints to 20 different plumbers, they would come up with 20 different layouts and they could all be correct.
