Is it possible to treat rusted iron water pipes or do they have to be replaced?

Sorry, but they need replacing. Not the whole lot straight away, but if one piece has gone the odds are the rest will need doing sooner than later

If you could remove the rust, you would have holes in your pipes, and if you could coat the insides of the pipe, you would not be able to drink the water. So the answer is no.
