I have perfectly fine water pressure whenever I turn on cold water, but turning on the hot water immediately reduces the water flow in the shower and in all the faucets. What causes this and how can I fix it?

Is sounds like you have ferrous metal piping that could have deposits restricting flow (volume) and thus causing a pressure drop. Have the piping checked by a licensed plumber to see if it is this condition.
