Leak outside from inside kitchen drain

I have a leak the is about 2 and 1 half feet from kitchen drain pipe. It comes out between house siding and cement foundation, when I run dishwashere or water in the kitchen sink. Also notice that my countertop in kitchen has shifted down toward where water leaks to the outside. Thank you for letting me send u my problem!
